Patient Access to GP Summary Record

Patients can view online a summary of their patient record, outlining the following information:

·         allergies

• drug sensitivities
• acute medication (issued in the last 12 months)
• repeat medication (current and past)

Should you wish to access a summary of your patient record please contact the surgery to obtain your own personal login details.

Patient Access to GP Detailed/ Coded Record

Patients can view online a summary of their patient record, outlining the following information:

·         allergies

• drug sensitivities
• acute medication (issued in the last 12 months)
• repeat medication (current and past)

Patients will be able to view online, in addition to a summary of their patient record, detailed coded information held within their medical record from the date the request has been made.

Examples of detailed coded information:

·         Results – (eg normal ranges of blood results)

·         Values (eg blood pressure)

·         Immunisations

·         Problems/ Diagnoses

·         Procedure codes (medical or surgical) and codes in consultations

Requests to view detailed coded medical records will be authorised by a GP on a case by case basis. If approved access will be made via the Systmonline patient login.

Summary Care Record (SCR)

This service allows other NHS health services (e.g. hospitals) to view patients’ personal information (i.e. name, address etc) and their health records relating to any allergies they may have and medications they take.

Patients can opt out of this service if they wish. Please contact the surgery if you wish to opt out.

Electronic Transfer of Patient Records (GP2GP)

The GP2GP facility provides the ability for patient records to be safely and quickly transferred, electronically, from one practice to the next as a patient moves.

This will allow the patient’s new GP to have more accurate patient records and medical information at the first consultation.

This practice will utilise this facility whenever possible.

Surgery is compliant with this facility.
